She really is sick? Do I need to contact her family?

I am visiting a friend for a while and I have noticed the following: I am concerned that my friend is quite ill and attempting to hide it.I do know she is diabetic. Stays in bed all day.Abuses her medication. Only gets up to eat. Rarely urinates, has had two bouts with uncontrollable bowels, complains of dizziness, has fallen twice and said her sugar was low. Difficulty with speech Has a hard time getting to the right word. Somewhat forgetful. When I bring all of this up, she says "I'm fine".
